Palliative Care House Call Cancer Patients
Dr. Hamilton is a family physician experienced in managing palliative care patients in the hospital and within their home environments.
As part of his core family practice, Dr. Hamilton is able to manage several palliative care cancer patients within their home environment.
Occasionally, Dr. Hamilton will have openings in his practice to accept the care of palliative care house call patients.
Under what circumstances will Dr. Hamilton help me to manage my disease symptoms within my home?
1. You must be a palliative care (cancer) patient wishing to be cared for at home and requiring house calls.
2. If you have a family doctor that requires assistance with your care within the home, your family doctor may request a consultation for this service from Dr. Hamilton.
3. Dr. Hamilton must have availability to care for you when the request is made.
4. Your home must be within Dr. Hamilton’s geographical palliative care house call zone. To view this zone, please see – palliative care house call zone.
5. For patients without a family doctor, you will require a referral letter from your oncology physician or treating physician requesting a palliative care consultation from Dr. Hamilton.
